This design can develop the same grinding or gritty feeling from two very different causes: genuinely worn or pitted bearing races and balls, needing a rebuild with fresh balls and possibly a race replacement, or simply a cone that's been adjusted too tight or too loose, either of which can produce roughness or play without any actual bearing damage.
How to tell adjustment from real wear
Check for play first: grip the wheel rim, with the wheel still in the frame or fork, and try to rock it side to side. Noticeable play alongside the grinding feeling suggests the cone is too loose, which is a simple tension adjustment. If there's no play at all but the wheel is stiff, notchy, or hard to spin freely, the cone may be adjusted too tight, which can be corrected the same way.
